Post Hospital Care
Returning home from the hospital is not easy for seniors and disabled adults. We provide post hospital care to help seniors and adults transition back home. Our caregivers provide assistance with ADLs and ensure that your loved ones follow their discharge instructions to help their recovery. We will also ensure they get the rest they need while we help take care of tasks around the house.
Companion Care
Our caregivers will be more to your loved ones than providers of personal care and assistance. We will pair your loved one with a caregiver who will also be a companion and help alleviate feelings of loneliness and isolation. Our caregivers get to know your loved ones and build personal relationships that include friendly conversations and fun activities.
Alzheimer’s and Dementia Care
Seniors suffering from Alzheimer’s and other forms of dementia require consistent care, and it is best for them to receive care in the familiar and comfortable environment of their home. We provide effective care for those suffering from all forms of dementia that includes personal care and assistance as well as positive reinforcement to counteract common behaviors of dementia patients.
24-Hour Home Care
Seniors and adults with disabilities who need around the clock care can get constant care and supervision with our 24-hour home care services. With this care, we will make sure there is always a licensed caregiver in the home to provide personal care and assistance and respond to emergencies. These services are ideal for those with illnesses or chronic conditions who require constant care.
Hospice Support
Our caregivers can provide additional support to those receiving hospice care, including personal care, companionship, and help with certain household tasks. Our goal is to ensure the comfort and quality of life of your loved ones so they can live their final days with dignity.
Veteran Home Care
Veterans and spouses of veterans who are eligible to receive benefits from the Department of Veteran’s Affairs (VA) can put these benefits towards the cost of home care. Assisting Hands Home Care is an approved provider of VA home care and works with eligible veterans and spouses to ensure they receive the care they need.

What to Expect When You Call Assisting Hands Home Care

Client information documentation
- We start by gathering information about the client who will receive care. This includes their personal information as well as information about their health and care needs.

Formal in-person intake
- We will then meet with the client and their family to learn more about them and their care needs.

Nurse assessment
- New clients are assessed by a nurse for more information about their health issues including illnesses and chronic conditions.

Develop care plan
- The information gathered is used to create a comprehensive care plan that accommodates the unique care needs of the client.

Assign caregiver
- We will assign a caregiver to the client who we feel is best suited to accommodate their needs within their schedule.

Introduce caregiver to client
- The client will meet their caregiver who will then begin administering care.
